AFGE Enrollment by Annual, Semi-Annual or Quarterly Direct Bill

You have elected to pay your premium annual, semi-annual or quarterly by check or money order. Please follow the below instructions and complete the enrollment form for submission.

1. AFGE CareerGuard® Enrollment Form, print, and sign.

2. A check for your first modal payment – Annual, Semi-Annual, or Quarterly.

3. Mail the completed Enrollment Form, along with the 1st modal payment to the address below:

Your coverage will begin on the day we receive your enrollment form and check.  Your payment will not be pro-rated for any partial month of coverage if received other than the first of the month.  If you want your coverage effective the first of the month following enrollment, please include a note with that request.  A renewal bill will be mailed 30 days prior to the expiration of the initial coverage period.  You can then renew by credit card online or mail a check.  Your Certificate of Insurance will be mailed within 10 days of enrollment.
